Direct Care Provider Job

Position: Direct Care Provider

Job Description: We are always looking for direct care providers to assist members with Attendant Care (ATC), Habilitation (HAH), and/or Respite (RSP) services. Each member has a unique care plan and requires different levels of assistance. Some members might only need one services and few hours while others need all services and more hours. Below is a description of each type of service.

Attendant Care: support the Member with activities that they, and their Responsible Person if applicable, have decided they need help with or that are documented in their plan, such as:

  1. Using technology or assistive devices as necessary

  2. Supporting nutrition, such as planning for preferred meals, preparing and cleaning up meals, and assistance with eating 

  3. Supporting self-care activities like bathing, dressing, grooming, using the restroom (e.g., bowel, menstrual, and bladder care);

  4. Providing mobility support or transferring;

  5. Completing domestic duties such as light housekeeping and laundry assistance;

  6. Shopping (except if member lives at family home);

  7. Providing supervision and using behavioral intervention techniques as applicable;

  8. Complying with medical needs and orders like attending to an injury or illness, monitoring skin integrity, providing first aid, referring the Member for medical care when needed, and assisting with self-administration of medication(s) or medication reminders;

  9. Attending to certified service animal needs;

  10. Assisting and supporting the Member to make choices;

  11. Assisting the Member with employment by supporting with personal care activities, health and medical needs, and providing mobility support as necessary on the job;

  12. Supporting employment or community activities;

  13. Providing companion care by transporting and/or accompanying the Member to appointments 

  14. In unusual circumstances, additional support might be required such as:

    • Heavy cleaning (e.g., washing walls or ceilings, cleaning the yard, and hauling debris); and

    • Assisting the Member in meeting their basic material needs for water, heating, and food.

 

Habilitation: provide habilitation service in accordance with the documented habilitation goals in the Member’s Planning Document. Documentation of results of the habilitation exercises in progress reports.​ Habilitation goals are teaching strategies that a Direct Care Provider is given to work with the Member. Habilitation goals are tailored to each Member's abilities and they are aimed at teaching them essential life skills. The goals can run the spectrum of skills from emotional or behavioral (e.g., learning how to communicate ideas or emotions)
to physical abilities (e.g., learning to dress themselves). Some Members may have few goals while others may have many. Progress to each goal is monitored by EAC Administration and reported to the DDD as required by their policy. Goals are adjusted based on mastery and changing needs of the Member.

 

Respite:​ short-term care to relieve caregivers. Respite is an interval of rest and/or relief to a family member or other persons caring for a Member, which is aimed at improving the emotional and mental well-being of the Member. The Direct Care Provider will:

  1. Supervise the member and meet their social, emotional, and physical needs.

  2. Ensure the member receives all prescribed medications in the ordered dose and time.

  3. Administer First Aid and give appropriate attention to injury or illness.

  4. Supply food to meet daily nutritional needs, including any prescribed therapeutic diets.

  5. Furnish transportation as needed to day-programs and appointments.

  6. Carry out any programs as requested by the Planning Team.

  7. Report any unusual incidents to the Division in accordance with policies and procedures.

  8. Ensure appropriate consideration of member needs, compatibility, and safety when caring for unrelated members.

Shifts: varies by Member's needs. 

Requirements, Training/Certifications: (will be provided by Eros and Care if needed):

  • Must be at least 18 years of age to apply

  • Valid Level I Fingerprint Clearance

  • First Aid and CPR 

  • Article 9 Certified Training 

  • Direct Care Worker Level 1 and/or 2, as applicable 

  • Prevention and Support, as applicable

  • Pass a criminal background test

Minimum Education:

  • High School Diploma or GED preferred.

Experience:

  • 1 year of prior experience working in a similar setting and capacity.

Essential Job Functions:

  • Ensure proper medication procedures are followed for the Members.

  • Ensure physicians' orders are followed correctly.

  • Responsible for communicating progress and issues in a timely manner to the appropriate member of management.

  • Responsible to maintain records/paperwork and submit documentation per assigned timelines.

  • Maintain a safe environment in keeping with Arizona Licensing Standards and Eros and Care Policies and Procedures.

  • Advocate on behalf of the Members and their families to ensure individuals with intellectual and developmental disabilities have a voice.

  • Perform daily job duties in a safe and healthy manner, including the use of proper techniques when lifting individuals, household items, and completing First Aid/CPR as needed.

  • Maintains confidentiality of information regarding Members and Eros and Care.

  • Compliant with attendance rules.

  • Cook meals and clean as assigned (Attendant Care)

  • Promote and maintain a collaborative and respectful working environment and a line of communications with all stakeholders of Eros and Care.
