Position: Group Home Direct Care Provider
Job Description: We are looking for direct care providers to assist members in a Group Home environment. A Direct Care Provider (DCP) is responsible for providing essential care to members in a Group Home setting. Each member has a unique care plan and requires different levels of assistance. Some members might only need help with meal preparation and general cleaning of the home as well as assistance during community outings. Others might need more, such as help with bathing, general personal hygiene, feeding, etc. All DCPs will help with general maintenance of the home, cooking, cleaning, medication aid, and documentation requirements (for example, habilitation reports, incident reports, shift reports, etc.). Eros and Care will provide all required training, including training to the specific member's care plan.
Starting Pay: $17.00, Drivers: $17.50
Shifts: M-F: Day 8am - 4pm, Evenings 4pm - 12am; Overnight 12am - 8am; Weekend 12hr/16hr shifts. Helper Shift: M-F 3 or 4pm - 10pm. Shift availability varies.

Must be at least 18 years of age to apply

Valid Level I Fingerprint Clearance (paid by the applicant)

Article 9 Certified Training

Prevention and Support Training

Pass a criminal background test
Minimum Education:
-
High School Diploma or GED preferred.
Experience:
-
1 year of prior experience working in Group Homes (preferred but not required).
Essential Job Functions:
-
Supervision of the Members' health, safety, and security to include assistance with Activities of Daily Living (eating, bathing, dressing, toileting, ambulation, and continence).

Encourage Members to develop and maintain positive interpersonal skills.

Ensure proper medication procedures are followed for the Members.

Ensure physicians' orders are followed correctly.

Responsible for communicating progress and issues in a timely manner to the appropriate staff and management.

Responsible to maintain records/paperwork and submit documentation per assigned timelines.

Maintain a safe home environment in keeping with Arizona Licensing Standards and Eros and Care Policies and Procedures.

Advocate on behalf of the Members and their families to ensure individuals with intellectual and developmental disabilities have a voice.

Perform daily job duties in a safe and healthy manner, including the use of proper techniques when lifting individuals, household items, and completing first aid/CPR as needed.

Maintains confidentiality of information regarding Members and Eros and Care.

Compliant with attendance rules.

Cook meals and clean the group home as assigned.

Promote and maintain a collaborative and respectful working environment and a line of communications with all stakeholders of Eros and Care.
